Loyola University Survey Administration Form
Office of Institutional Effectiveness (OIE)
 

This form is designed to collect essential information on all potential survey activities with Loyola constituents (e.g., faculty, staff, students, alumni). When completing it please provide as much information as possible about your proposed survey. The information will be reviewed by the Loyola University Survey Coordinating Committee. This committee is charged by the Provost to review proposals and make improvement recommendations to submissions. The committee will also work with the Associate Provost and Director of OIE as well as the OIE Director of Survey Research to ensure coordination of survey administration requests with pre-planned institutional surveys. 
 

Of high priority to the Committee is (1) the relevance of the survey research and how it contributes to monitoring continuous improvement  (this may also include surveys that identify previously unknown nuances of faculty, staff, and student experiences that Loyola can then plan thoughtful programming or policy and practices changes), (2) a well-established data management plan as a protection of the privacy of survey participants, (3) a dissemination plan to ensure the data are used appropriately and responsibly, and (4) the timing of this survey with other surveys as a means of avoiding survey fatigue.  
 

Examples of the types of work that would not require the submission of this form include LUC employee exit surveys, compliance/specialized accreditation surveys, voting/polls, and post-event/activity evaluations. Surveys that are directly related to Loyola faculty sponsored grant-funded work must provide an IRB approval letter to the Director of Survey Administration prior to gaining access to Loyola constituents.  Please contact the Director of Survey Administration at bmasini@luc.edu if you are unsure if you should complete this form. 
 

The Committee will make recommendations to the Associate Provost of OIE as outlined below: 

  • Whether to provide full support to proceed with the survey on the proposed survey administration schedule. 
  • Whether to provide full support to proceed with the survey with a request to adjust the proposed survey administration schedule 
  • Request additional information before launching the survey (specifics will be provided) 
     

This information will be reported back to the lead contact making the request.
